You can have a US iTunes account if you're not in the USA, I don't personally, but it can, or at least used to be able to be done.
As to why Apple don't allow it the only reason a dev has ever given me was that it is something to do with accounting or law, which I don't buy.
I am UK based, and believe me it annoys me that I can't use these codes as I would have saved a lot of money over time, but that doesn't stop me from making them available for others.
One day I hope Apple will see sense and open up codes to other countries which will ease my bank balance, but until then I'm happy to offer them if developers want to give them out and if it benefits readers, albeit only some.
'Tis rubbish that Apple only lets you redeem vouchers with a US account. I'm lucky in that I have an address in US and UK but even then it's a PITA having two accounts to look after just so I can use gift vouchers and promo codes. E.g. you can't update apps bought on one store when logged into the other - you have to log out and log in to the other one. It takes about 3 entries of a password just to update an app you already own.
Annoying move Apple, but hooray to Palm Sounds for offering promotions, even if my sub 1 minute response to the last one didn't net me the prize... (prolly Hotmail's fault).
